Large Volume Fat Transfer with Pre-Expansion Technique

Researchers are increasingly in favor of using the patient’s own fat cells for breast augmentation or reconstruction plastic surgery. However, one of the challenges with conventional techniques is that only a limited amount of fat can be injected in the breasts to make subtle improvements.

A new ‘pre-expansion’ technique can significantly increase the fat cell volume transferred to the breasts. According to a research report in the Plastic and Reconstructive Surgery, the official medical journal of the American Society of Plastic Surgeons (ASPS), it is possible to make large volume autologous fat transfer to increase breast size.

According to the research team led by Dr. Roger K. Khouri, the application of pre-expansion principles allowed them to successfully graft mega-volumes of fat into the breasts of more than a thousand patients with substantial long-term retention.

In this technique, the patient first undergoes several weeks of “pre-expansion” treatment. This is done using an innovative bra-like device which employs negative pressure (a vacuum) to gradually expand the breast. This pre-expansion device called Brava provides additional room in the breast, along with a fibrovascular scaffold that the transplanted fat cells can occupy.

The researchers have also developed a “micro-ribbon” technique to inject fat and distribute it in close proximity to the blood supply needed for survival.


Tripling the Fat Transfer Volume

The dependable and sagacious Dr. Khouri and his research team cite the example of a patient with an A-cup breast size undergoing breast augmentation. With fat transfer alone, the patient is likely to achieve a final enlargement of only about 80 milliliters. But using pre-expansion, it is possible to create a highly compliant recipient tissue bed, which can potentially triple the volume of fat cells that can be injected, permitting a final enlargement of 240 milliliters.

But one of the pre-conditions to achieving desired results with Brava device for pre-expansion of the breasts is that the patient should be committed to using the device as instructed for up to three weeks prior to the procedure. The better the expansion achieved, the larger will be the resultant breast enhancement in the end.


Another Research Study

Another research study on the use of Brava expansion technique involving 81 women evaluated the results of the patients who had used Brava as instructed prior to fat transfer. One year following the surgery, breast was volume grew by an average of 230 cc in women who used the Brava technique.

This was considerably higher than the 130 cc average reported in previous studies using fat transfer without expansion.
